Provides moderately-intensive, solution focused, home-based family intervention, life skills education and supportive services designed to generate solutions to problems that threaten family stability and preservation. This service is provided for 90 days with the opportunity to extend an additional 90 and involves a minimum average of 3 hours face to-face time with the family each week.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Responsibilities associated with this job will change from time to time according to the Agency's needs or contract. More specifically, the incumbent may be required to perform additional and/or different responsibilities from those described below:

  • Adheres to Families Together Building Solutions models of service, using solution-focused interventions.
  • Makes collateral contacts including but not limited to: medical care, teachers, mental health providers and links families to community resources.
  • Provides 24/7 emergency availability for the 8 client families on the worker's own caseload and works a flexible schedule including some non-traditional hours (evenings, weekends, holidays).
  • Teaches families parenting skills, coping skills, and skills for living.
  • Prepares reports within appropriate time lines, and maintains documentation on all cases.
  • Assists families in obtaining concrete services, such as housing, transportation, house cleaning, etc..
  • Collaborates with DHHS CPS referring workers and Pathways to Potential Success Coaches.
  • Functions as part of a team in weekly meetings and other activities.
  • Works on COA related directives for FTBS.
  • This position will be knowledgeable about and actively support 1) culturally competent, recovery-based practices; 2) person-centered planning as a shared decision making process with the individual, who defines his/her own life goals and is assisted in developing a unique path toward those goals; and 3) a trauma informed culture to aid individuals in their recovery process.

A Bachelor's Degree in Social Work or a related Human Service field is required. Experience working with families is highly preferred.

Excellent verbal and writing skills are required, along with the ability to communicate verbally with clients from many different socio-economic and cultural backgrounds.

Ability to stay calm in emotionally charged situations and to problem-solve in crisis situations is required. The ability to negotiate goals with clients and workers is critical. The ability to understand and constructively interact with other agencies and the legal system is important.

While performing the duties of this job, the employee must be able to travel to all areas within Kalamazoo and/or Calhoun Counties. Must be able to transport clients in own vehicle. A small percentage of home visits may need to be made in the evening, on weekends, or holidays. The employee must be able to climb stairs and enter home environments that may be cluttered or present other physical obstacles. The employee must be able to visually assess the safety of the environment and the physical well-being of children in the home. There may be exposure to dust, cigarette smoke, pet dander, and other typical household conditions.

The noise level is usually moderate.
