Provide case management services that empower youth, young adults, and families to participate in a community environment.  Serving families with a child (through the age of 18) who has been diagnosed as having Serious Emotional Disturbance (SED) or Developmentally Disabled and young adults (18-26) with a Serious and Persistent Mental Illness (SPMI).  Services are individualized and efforts are focused on educating and empowering consumers so they may achieve their goals, direct their own lives, increase their natural supports and resources and are less dependent on service providers.  Case Management seeks to help individual/families better understand problems and partners with individuals/families so they may work on creating alternative solutions, providing coordination and monitoring of service delivery to assure the consumer has access to appropriate treatment and services.

Minimum qualifications:
MSW/MA in human service related field, BSW with work experience may be considered.  State of Michigan limited or full licensure LBSW, LLMSW, LMSW.  Minimum one year experience in the examination, evaluation, and treatment of youth/families required at a Masters level and three (3) years experience at a Bachelors level.  Strength-based approach, teamwork, excellent organizational, and communication skills are a must.  Valid MI driver's license and driving record which complies with agency policy.  Ability to regularly provide out-of-office service in community settings in non-structured, unpredictable situations.  Ability to implement physical assistance/intervention as warranted
